      Please navigate to your admin dashboard and go to CatalogX > Settings > Enquiry Forms. Under the ‘Enquiry Form Field’ section, ensure that the necessary field sections are enabled as needed (https://prnt.sc/1AJ2MAoQBo3p). Then check the enquiry form again.

      Hi @olan, Sorry to hear you’re facing this issue.

      It seems there might be a theme or plugin conflict. To troubleshoot, please deactivate all plugins except WooCommerce and CatalogX, and switch to a default theme like Twenty Twenty-Four or Storefront to see if the issue persists.

      If the problem continues, kindly create a staging site that replicates your live site where the issue occurs, and share the staging URL along with temporary admin and FTP access. This will allow us to investigate the issue on your end.

      We investigated the issue on your site and found that the permalinks were not set correctly, which caused the problem. We have reset the permalinks on your site (see screenshot: https://prnt.sc/l5o5SeyPil8a), and now the settings page is displaying as expected. Please make the same adjustment on your live site and check if the issue is resolved.

      We didn’t change anything. Our permalinks on the production site are the same as your screenshot. Maybe we are doing something wrong.
