We have checked this on our end and the downloaded CSV file is showing the vendor application data accordingly. Now on your end kindly check the flow again carefully by. deactivating plugins other thatn WooCommerce and MultVendorX and also switch to some default theme such as twenty twenty four or storefront.
If still the issue persist please do create a staging site that should be an exact replica of your live site and share the staging site url, temporary admin and FTP access of your staging site with us so that we can check the issue on your end.
Also while sharing the access don’t forget to mark your reply as private.

CopyRegarding the modifications in the email templates, you need to add customisations on those by overriding those template files. For any further assistance regarding the customisations in these templates kindly get in touch with the wooCommerce team. They will be able to assist you further accordingly.
